5 Things to bring to your next DOT Physical Exam.

5 Things you can do to avoid any delays when going for your DOT Physical Exam

 

1) ID: Your valid driver’s license and /or other photo ID

2) SLEEP APNEA: Bring your most recent CPAP compliance report. Report must show at least 70% of usage.

3) GLASSES / HEARING AID: If you wear corrective lenses for driving be sure to bring your eye glasses or contact lenses to your appt. Bring a working hearing aid if applicable. 

4) Medical Clearance / Medications: Bring your Medical clearance letter from your primary doctor or specialist for any medical conditions being treated, or recent illness, injury, or surgery. Bring a list of your prescribed medications. 

5) Current FMCSA Waiver or Exemption Certificate or Skills Performance Evaluation if applicable.
